I have 3 companies in QB Online. One changed to a new layout months ago, where Transactions are Confirmed instead of Accepted, among other changes. The others have not?

There's an update in QuickBooks Online where some of our users are able to accept downloaded banking transactions by clicking Confirmed. 

Since it's not fully implemented, you'll still see the Accepted or Add button when confirming transactions.
