I am trying to find QBO's DD form and cannot. The link in the attached message does not work (in my QBO Accountant software).

Thanks for joining us here.  After you've set up direct deposit and verified the test transactions, you're ready to set up your employees.  You'll need the Direct Deposit Authorization Form, for your staff.
