Adding free text to templates

I'd like to add a couple of items of free text onto my packing list template. For example:

 

Picked by:

Approved by:

 

The packing list hard-copy is used by warehouse staff to pick, pack and ship. This document will then be filed as a record for traceability purposes.

 

How can I add this free text?
