Bank balance wrong in balance sheet

Can someone please advise what I might be doing wrong. 

 

I reconcile my bank every month, but now the accountant has run a balance sheet at the end of the year 30/11/19  and  the bank balance  is showing a completely different figure to what the reconciled amount was at that date. 

 

I don't know where to start to try and fix this so any help would be great.
