Customising Forms

Hi everyone,

 

I am using Sole Trader, have just moved from Self Employed version, I am trying to customize my invoices/quotes etc but whenever I click on customise I get this - https://qbo.intuit.com/app/appfabric-defaults/not-found - Not found

 

I want to put my company logo on the forms, I have added the logo in the account settings - but it is not appearing on the logo and I cannot see where else to add/modify the logo on the form.

 

Help pls...
