Where does the address that is entered on the direct debit form come from?

The form sent to our customer automatically adds an address to the form after she enters her bank details. This is not her current address and she is concerned about the error. He address in QuickBooks is correct.

The address on the form could you  provide a screenshot showing how it appears(if you black out  any personal details.We have been advised by our Go Cardless tier 2 support team the address would usually be the bank address we just want to confirm if that is what is happening in this instance
