Thank you for reaching out to the Community, METS2025. I'm here to assist you in resolving the error message you're encountering when filing the NYS-45 Form in QuickBooks Desktop (QBDT).

This error typically occurs when there is a mismatch between the filing period and the selected quarter. Let's ensure all employee payroll data is entered correctly, including wages, taxes, and other relevant details. Missing or incorrect data can lead to discrepancies in Schedule C and the quarter selected on the form are correct by recreating it.

Before proceeding, let's make sure you have the latest tax table updates. To have an accurate rates and calculations for federal and supported state taxes, payroll tax forms, and e-file and e-pay options.

Here’s how to access the form:

  1. Navigate to Employees in the top menu bar and select Payroll Center.
  2. On the Payroll Center page, click on the File Forms tab.
  3. Disregard any saved or draft forms created from your initial attempt to pull up the NYS-45 Form.
  4. Select NYS-45 and NYS-45-ATT under the Form column and click on Create Form.
  5. In the File Form pop-up, enter the Start Date and End Date.
  6. Click OK.
  7. When the form opens, ensure that the Quarter 4 box is selected.
  8. Continue with the process to file the form.

If the issue continues, I recommend reaching out to our QuickBooks Desktop Payroll Team. They have the necessary tools to access your account securely and investigate why the NYS-45 form is not populating the amounts for the quarterly return.

Furthermore, I'm adding this resource for future reference on how to run, print, and customize payroll reports in QuickBooks Online Payroll and QuickBooks Desktop Payroll: Run payroll reports.

I'll be around to help if you need anything else about filing NYS-45 form in QuickBooks Desktop Payroll. Don't hesitate to leave a post and the Community team and I are ready to assist you.

Thank you for your response. This problem started after a recent payroll update.
First of all in the form selection menu, instead of NYS45 having a Quarterly status, it has ANY.
Second,  once the form with the correct dates and quarter is selected, it fails to populate schedule C with the employees data and other relevant details and totals. This results in an ongoing error.
I have reached out to QB Payroll Customer support and was told that they are aware of this problem with the update and their engineers are working to fix it with a new update to correct this problem.

Same issue. When will there be a fix?

January 22, 2025

Same problem - just went to file the 4Q NYS-45 and NYS-45-ATT - Quarterly Combined Withholding and it is not loading the NYS 45 ATT attachment which is what is causing the error to show that the totals don't correspond. As someone else mentioned, in the Payroll Center > File Forms tabs where you select these forms under Filing Period column it is not listed as a "Quarterly" form anymore, now says "Any" and you have to select the start and end dates.  Don't know if that's part of the issue.  But NYS-45-ATT is not showing up and I even downloaded and printed it out and the ATT was not part of it.  I also just did the payroll update right before trying to completed this task.  I restarted Quickbooks twice and tried Payroll Update twice just to be sure and it did not change the issue.

I'm having the same issue. It won't even let me type the employee info or override.

Hi QuickBooks, the problem is on your side and you know it. Please just fix the issue.

January 24, 2025

The problem has been fixed. Well, they didn't inform you anything about it.

You will need to do one single "Payroll Updates" in "Payroll Center". Not every company file needs to be updated. Hope this will help everyone.
