Ontario WSIB setup

I'm trying to set up WSIB in Quickbooks Online in Ontaio.  When I go to Preferences --> Payroll Section --> Provincial/Territorial Taxes - ON,  I get to the section Ontario (WSIB) Workers' Comp Remittance Frequency

 

From there, I have the Remittance Frequency Section with a drop down menu that lists Quarterly, Monthly and Annually.  I have to remit quarterly so I select that from the drop down but when I then select OK I get the error message: Please enter a valid date in the format mm/dd/yyyy.

 

There is no date field on this form so I don't know what to do next.
