I am paying a monthly fee for QuickBooks online. Should I have to pay a monthly fee for the app on the Apple store. QuickBooks says online no additional fees for app.

The answer is no. You don't have to pay for anything when using the QuickBooks Online mobile app. All you have to do is download it on the App Store and sign in using the same login credentials.
