Unable to select "Pre-tax insurance premium"

Hi,

 

I'd like to set-up a pre-tax insurance premium for our staff and I'm unable to select that option (the default is set to Taxable insurance premium). I have tried every permutation of "Flat amount" and "Percent of gross pay", multiple browsers, ad-block on and off, etc. No matter the approach, a black circle with a slash prevents me from choosing "Pre-tax insurance premium." How can I solve this issue? Thanks!
