I clock in but after an I hour it was not clocked in

Hi,

 

I was sure that I was able to clocked in, but I was told that I was not clocked in for 2 or 3 hours and it shows in the record that I was not able to clocked in between that time, but when it's the end of my shift, it did not show clock in, but clock out.
