It's not possible to create or edit the header of the template in QuickBooks. The header is already set as a default setting in the invoices, even if you're still using the old layout.

However, if you're referring to the form name in the header, it is possible to change it. You can create a custom template and modify the form name. Then, you can use this template when creating a Change Order.

Here's how:

1. Sign in to your QuickBooks Online account.
2. Select the Gear icon, then click on Custom form styles under Your Company column.
3. Choose the invoice template you want to modify, then click on Edit.
4. Once routed in the invoice template field make sure to remember the template name and number.
5. Tap on Content in the invoice template field, then select the pencil icon on the right side to edit the section that you want.
6. Hover your mouse until you reach the Form names, and change the invoice to Change Order. 
7. After changing the form name from Invoice to Change Order, hit on Done.

When opening the invoice, make sure to select the modified template by going to the Design section. After that, you can click the Print and download button to verify if the changes apply.

     • Contract sum will be increased/decreased by this change order.
     • New contract sum including this change order.
