Question

In a $5,000,000 company who should categorize the bank and credit card expenses? Should it be the person that is entering the bills or a different person?

It can be the same person that enters the bills, but if you want solid internal controls, a different person should review the cc charges and pay the statement balance.  Same with bills - one person verifies the bills are legitimate and enters them, another reviews and pays.  Bank charges should be entered by one person and reviewed by another.  I had a couple of similar sized businesses and we employed a bookkeeper.  The bookkeeper entered the bills and cc charges and reconciled the bank account.  I reviewed the bills and cc charges and paid them and also reviewed the bank reconciliation.  It took a couple of hours per week max.
