Desktop version Estimate Template Customer Name Fields

I'm trying to get our users to properly fill in the customer information fields instead of manually typing. I want to run customer reports and the way they were/are entering data, I can't.  This is the first step in cleaning up the database. 

I can get customer phone number, email and address fields.  However, I can't find the customer full name field to add to the estimate template.  This is the desktop version.  See attached - those are the fields I want to include in the template.  

Can someone help me please?  Thanks.
