Question

I have entered a specific receipt multiple times, it comes up in search. However it does not show up in expenses or under reviewed receipts.

This also does not appear on graphs on home page. How do I resolve this issue and keep it from occurring again?

To get started, I suspect you may be experiencing browser-related issues that can cause issues such as the one you've described. This was actually the first thing I tried, Mozilla firefox browser, also updated the browser. No dice :(.
