Louisiana L-4 taxes form

I have an employee that filled out Louisiana L-4.  Block A entered "0" and Block 3. chose "No exemptions or dependents claimed".  In QuickBooks the options are Single, Married, or Exempt, which do I choose?

According to the information your employee provided on the L-4 form, you'll need to select the Exempt option in the Filing Status section when setting up their state tax details. I've outlined the following steps to guide you through the procedure:

 

  1. Go to Employees, then select Employee Center.
  2. If you’re adding a new employee, click New Employee and double-click their name from the list if they're existing.
  3. Choose the Payroll Info tab and hit the Taxes button. 
  4. Click the State tab.
  5. In the State Subject to Withholding section, pick Exempt in the Filing Status field.
  6. Hit OK twice.
